Please select the forms you need to fill out and follow the instructions below to fill out the form and submit it to our clinic email.
In order to edit and submit your forms from your iPhone you will first need to download the Adobe Acrobat Reader app from the app store. This is a free app that just functions as a pdf viewer. Open the app and create an account. Again all this is free.
Once you have downloaded the app and created an account, select the form you need from the list above.
It will open in your browser and you will not be able to edit or submit it from there. Below are examples of how it will look in Google Chrome and Safari for iPhone.
If you are using Safari, the default browser for iPhones, you will see the document and a small share icon at the bottom of the page. Select this button and then click the Acrobat icon from the list.
For the Google Chrome web browser for iPhone your screen will look like the following. Select the open in option at the bottom and then select the import to Acrobat option.
Then both Chrome and Safari users will select the fill and sign option and this will open the form in the Acrobat app where you can edit it. Once you are done filling out the form you will select the share button at the top of the screen.
It is now time to share you completed form with our clinic email address. In the Invite People By Email Box enter firstname.lastname@example.org and then select Done in the upper right corner.
Next you will hit send and we should receive your completed forms in moments.
