Home Health Services


What is Home Health Care?

Health care provided in your home by nursing professionals under the orders of your physician.


Who is eligible?

Any person in Cass and surrounding counties who is essentially homebound with a medical problem, and who is under the care of a physician, may be eligible.


What is the cost?

Fees are covered by Medicare, Medicaid, health insurance or private pay. Please call—our qualified staff would be happy to assist you in determining if home services are covered by your insurance.


How can you receive home care?

Ask the discharge planner/ social worker at the hospital. Ask your primary care physician. Call the Cass County Health Department at (217)452-3057.


What are the advantages of Home Health Care?

  • Allows the patient to recover at home in familiar surroundings with his/her family and friends
  • Less expensive than hospitalization or nursing home
  • Can prevent the elderly from early nursing home placement by providing support and help in the home


Community Health Care


What is Community Health Care?

Community Health Care is a private pay program for those individuals who otherwise would not qualify for Home Health services.


What services will you receive?

The services provided through this program include

  • Blood pressure checks
  • Trimming nails
  • Personal Care
  • Medication/insulin setup
  • and many others


What is the Cost?

Payment is a sliding fee scale based on total monthly income.

$0 - $999 20% of cost

$1000 - $1499 25% of cost

$1500 - $1999 40% of cost

$2000 - $2999 60% of cost

$3000 - $3999 80% of cost

$4000 + 100% of cost
