Cass County Health and Dental Clinics outsource our release of information process to HealthMark Group.
Patients
HealthMark fulfills all patient requests for release of information. First time users are required to register for an account to access the website https://requestmanager.healthmark-group.com/register .
Once registered and logged in select "Submit Request" from the menu options and enter all required fields to submit the authorization electronically. HealthMark will process your request and provide notification via mail or email once complete.
HealthMark fulfills all patient requests for personal copies at no charge to the patient. A complimentary copy of your record will be made available for you to download through HealthMark’s website at https://requestmanager.healthmark-group.com/register.
When calling to check status on a previously submitted request, please allow 3-5 business days for processing. Patients may then contact HealthMark, at 800-659-4035 or by email to status@healthmark-group.com.
Medical and Non-Medical Providers
Please submit a HIPAA compliant authorization for release of information to our office via fax at 217-452-7245 or mail to: Cass County Health Department 331 S. Main Virginia, IL.
When calling to check status on a previously submitted request, please allow 3-5 business days for processing. After 5 business days, status inquires may be submitted by email to status@healthmark-group.com .
Home Health, Hospice and other
Cass County Home Health, Cass Schuyler Area Hospice, and other records are often in a hybrid format (paper and electronic) which requires additional time to process.
If you would like to request patient information from any of these programs, please submit a HIPAA-compliant authorization for release of information by completing the HIPAA Authorization to Release Information and print / complete and mail / return / fax to:
